Credit Card Termination Form

Just thought this might be useful.

Applicable for general plastics termination in Malaysia. Fill up your own details and fax over to the credit card company.

You just need to make sure that the credit card company received the fax.

Max, though I have 2-3 active credit card, I made it a point to only use one card at any one time. The other 2 are just for back-up, in case I need to cancel the one that I'm not happy with.

Hi… thanks for a format. However, if in word format is better because easy to edit.

Could you please also list up fax no for all Malaysian bank credit card? It will be a great favour.

Hi Tom,

The form is made in such a way that you just need to print the form as it is, and then fill in the blanks. The details are all the credit card companies need to proceed with the cancellation.

Hehe…sorry, I can come up with the list of fax numbers, but updating the numbers to ensure its relevancy is going to be too much trouble. I'll leave that part to the cardholders ;-)
